SilverSneakers® is a health and fitness benefit designed for Medicare-eligible individuals. It’s included with many Medicare Advantage plans and select Medicare Supplement plans. Silver & Fit®
Silver&Fit® is a program that health plans extend to eligible Medicare Advantage/Supplement beneficiaries and group retirees to enhance their quality of life. Renew Active
Renew Active by UnitedHealthcare® is a fitness program for body and mind to help you stay active, focused, and connected either at a gym or from home at no additional cost. Renew Active members are eligible through participating Medicare or Medicaid plans. Eligible members receive access to a no-cost fitness membership. Prime® Fitness
Prime® Fitness is a fitness benefit offered by select health plans and employer groups to individuals 18+ years of age. Please note, some Prime clients refer to this benefit by other names (e.g., Well onTarget, Fitness Your Way®, Walton Life Fitness Pass). Eligible members may receive access to a low-cost membership. Membership Information
Memberships will be processed by credit/debit card or Electronic Funds Transfer (EFT). There are no long-term contracts. If you would like to cancel your fitness membership, please complete this form or visit the Fitness Center to cancel in person. It is the sole responsibility of members to cancel their membership. All cancellations require 30 days’ notice prior to your next billing cycle. Monthly billing memberships have no expiration date. Retroactive cancellations are not allowed.
